What's the best way to cut out hair from a background?

Use the Lasso Tool. Make a quick selection around the part you want to crop. Don't worry about making it perfect just yet. Then press SHIFT and keep it down as you continue to make the selection perfect. SHIFT ADDS TO THE SELECTION. CTRL REMOVES FROM THE SELECTION. That should do it.

This is a very vague question, what is the background? plain or cluttered?
How much hair? lots? or a few wisps? If a girls hair was blowing in the wind, would it appear strange if it was simply removed from the photo?

You don't need a steady hand. Make QUICK selection around the hair then zoom in as far as needed and using the ALT key to remove selection and the SHIFT key to add to the selection you do not need a steady hand. And like Jack pointed out it could be easier depending on the banner. If it has a white bg or any bg that is the same overall and is very different from the image being cropped you can use the MAGIC wand tool and it automaticly selects the image.
