I need to make a page where you can upload a ringtone and send it to your phone... something like this:

but since I haven't worked with forms much I have no clue how I can set it up and if I can do it with html.

It's easy to upload it to a site, but how do you going to sent it to a phones? You'll need a WAP technology or smth like this...
