I am not sure if this is the proper forum for this question, if it is not then please let me know and I will move my question there.

I have created an online application for a corporation and I need to have the results emailed to a specific address. The server originally had a free script from Matt's Script Archive on it that they used for their old application and for a feedback form but they wanted both items changed to better suit their needs.

The only changes I made were @recipients to be my email address
@referers to include a new domain since they recently changed their name.

The HTML can be found at : Online Application - Party USA & Halloween USA

Any help or direction is greatly appreciated. I was hoping to avoid writing my own cgi or pl script for this. I should also mention that the premissions on this form are set properly to the best of my knownledge and that there is another form running on this server that works prefectly and that can be found at
http://www.halloweenusa.com/feedback.html

Thank you in advance!
