How do I get the text in the left table field at the top and on left side?

About your question, change: td.leftfield {padding-right: 1cm }

To:

td.leftfield {
padding-right: 1cm; 
vertical-align: top;
}

Does the stylesheet apply to printwork ? It doesn't make a lot of sense to use centimeters if you're designing for a screen. Thought I'd mention it
