How would you go about writing a T&A document? What exactly is it used for?

In it you have to outline just what you are going to do and when you are going to do it.

One way would be to look at the T&A of other companies in your industry to piece together the kind of document that fits what your doing. A lawyer might also be helpful if your looking to craft something more along the lines of an AUP(authorized use policy), or a EULA (electronic user licensing agreement).
