Hi All

Do any of you have (or know where I can find) a work authorization form for web design?

We normally ask our clients to sign a contract if the work is over $1000. However, for smaller projects we'd like a cookie-cutter form that people can sign manually or digitally.

Basically it needs to be similar to the form you sign in a garage or hospital, agreeing to be liable for the work they'll do for you.

There are several different contract templates on the microsoft templates site: http://office.microsoft.com/en-us/templates/

You may need to join to view or save them. It was free when I did a few weeks ago. Chance of finding an "out of the box" solution that fits your needs perfectly are pretty slim.
