Add'ing a New field and colum to this current php order form script : http://www.dyn-web.com/scripts/orderform/
The colum is going to be called "type" would like it to be a drop down menu containing the options a-j where a = $5 b = 10$ c = $15 d=$20 etc etc..
example: if drop down menu b is selected on product 1 it will add $10 to the total boxes
