I need a PHP/MySQL backend system to capture data from members of the public and pass it on to my clients. Only basic backend work is required - I am able to do the design.
Basically, the form needs to be able to capture several fields of form data from the public, and an uploaded file where necessary, from a single page. There will be several of these pages for different clients of mine. Once the data is captured, my client needs to be able to log in, see a list of submitted forms, and click on them to get the full page of details of what was submitted and the ability to download files where necessary. Then, they need to be able to select, delete and sort submissions and send emails to those selected.
The sticking point is that clients need to be able to see only the data that was captured for them. It may also be useful if they could sort their results by different fields and highlight the ones they are interested in. I have attached a descriptive PPT to give you an idea...
