I've got a contact form on my site www.whymwebhosting.co.uk but it doesnt work properly and I'm going to ditch it for a new form.
The form needs to fit the colour scheme if possible, but I definitely want it to have anti-spam capabilities e.g. asking user to type in letters of image or asking theme to add together several numbers, so that spam bots will not be able to use the form.
I'd pay in the region of £10 ($20) for something like this.
