I have got it at a mediamarkt, near my place. Relatively cheap, less than 80 CHF, which would make around 50 €.
And as for the service provider, I'd separate them if I where you.
I had problem once with an isp,and changing it forced me to change my voip phone number.
Having a separate provider would have avoided me that.
