Did you mean no ads displayed on your page? If that's the case then possible that google banned your adsense account.

How to get it working again? - Mail adsense team.
Did I lose it forever? - No, if Adsense team feels that you have done nothing violating its T&C. Yes- If it feels that you have violated T&C.

From what people have said in various forums, most people who ;ose their adsense accounts don't get them back
However some people have contacted adsense and had their accounts re-instated. If you are certain that you have remained within the TOS, then I suggest you contact your local google adsense representative and highlight your concerns

Chances are slim. Only you and Google know if you did something wrong. If you did do something wrong, banned forever. If not, contact them and see what they can do.

You can appeal to google with through adsense-support@google.com
but the chances of reinstating the disabled account is slim.
Correct the mistake, if you know, before appealing to google.
