It's a scam. It's basically saying to setup adsense on your site, so if the visitor doesn't take whatever action on your page, they may click on an adsense ad. Thereby offsetting your adwords cost.

You can do this without buying any product.
Just combine different action-options on a page -> affiliate links, opt-in form and adsense.

I'm curious... Is this the same doctor that has been using Google for free and make 300 mil..?
