This is not a contest, I will pay $50 for this.
I just need the two attached forms to be made fillable. I need two buttons added to them: a print button, and a submit button. The submit button will email the info to a specific email address I will provide you. It needs to emailed without using the user's Outlook. It will be hosted on godaddy.com so this will be the way it's emailed.
Please contact me before starting the job so I can let you know if you're the first one. I don't want to waste anyone's time by you creating the forms and someone else is doing it.
