Ok I have decide to add a "Click Here" button image where the form was and just point the image to an exterior form I have on the web.

Now the problem I am having with this is that after editing the image and adding the button, the image is not displaying correctly(I can see pixels thru out the whole Image), any tips?

I don't think "tacky" adequately describe such a monstrosity.

You can't honestly believe THAT image is going to illicit a positive response from your site visitors.
It's more likely to get the close or back button used a lot.

yes i agree, perhaps i went to the wrong designer, i need to learn to do this on my own.

I think you would be better off blinking your eyes to create the visual effect.
Otherwise you could do the old school method and make an animated gif in image ready.
