Do you want an opt-in form for one of your affiliate programs or just for a mailing list? Does it have to be a pop-up? Most people turn pop-ups off.

You gonna need some more details for get that done. e.g. how and where you gonna collect the data etc. Popup is not a good solution, rather have a page with the optin form on it.

Have one opt in form on the main page (non pop-up).

Put another one in an exit pop-up, which is less irritating.
