I would like a contact/feedback form for my website, but a little unsure how to make one , in the past i have used a hosted one but i would like my own with the following fields:

Name:

Email:

Comments:

And all the feedback to be emailed to contactus@bishopsgarthscience.co.uk

Is this possible?
