I am creating a page for a dept. at my university. What i'd ideally like is a way for the staff to enter info into various fields (as a search) and have it produce a result with some of the info from those fields entered into certain areas (i.e. in a document that has blanks)

I have no idea how to make a search like this compile then enter info like that.
