html form templates anyone??

Hi,

I know there are hundreds of places to get normal html templates (as in layouts) but does anyone know of anywhere to get some templates for html forms?

I have to create some rather large forms and I am looking at ways to have them laid out, it would be good to have some examples for reference.

I found EasyForm Software - haven't tried it, but it may help you.


For an Online Tool for Custom Forms, this may help.

Sorry Diane, I'm after HTML forms, not paper. I actually already use SmartDraw which is very good for producing paper forms, charts, diagrams etc.

However, I did come across this interesting site about Transform. Is this more in tune to what you need?

No worries.

Thanks for that. I actually already have the form processor and know how to produce the html form with no problems at all.

What I am looking for is something to help me create the layout of the forms, for example ideas on positioning checkbox groups, where to add hints and so on.

This seems quite obvious, however it would be nice to have some clean example forms to use as a guide.

I am a FrontPage user myself.

Don't really have any form templates, but if you need any help with form basics, you might check out my site. My tutorials are specific to FP, but the info still applies in terms of how to use the fields themselves.

I do have some form script examples you might find useful.

HTML forms are usually just inserted in template layouts, I doubt there would be places that concentrated solely on form templates. If it's for referencing, I guess the best way is to see the "contact us" of your favorite sites

Thanks, I have been looking at various sites to see their form layouts. Trouble is most sites only have a few fields (ie contact us) and I'm creating quite large, multi page forms.

Anyway, here is what I have created so far, let me know what you think and any hints or tips;

PS. It is not actually working (not linked to a script yet) and it is only a test for the layout so don't go looking at the code and commenting that I have not named form elements etc! It's just the layout.

recedo the form is looking pretty good so far. The textarea for the 'Profile' extends much further to the right than any of the other textareas or text inputs. Otherwise the layout is looking good to me.

I might also use separate text inputs for the address and offer a dropdown for the state. I think having all the components of the address separate makes it easier in filling out the form and will also be easier for you to sort out in the code as fas as what is what.

For example if you get an address sent to you as:

123 Main St. South Brunswick

Is the city South Brunswick or is the street Main St South?

Hi there,

Thanks for the hints. I'll change the address into seperate textboxes, I see how this would be easier, not sure why I put in a text area to tell the truth!

The reason the 'profile' textarea is larger than the others is because it will need to contain more text than any of the other boxes. The text in here is what is used to fill the 'from the band' sections on the bands profile page, see http://www.notsounknown.co.uk/nsu/pr...Died%20Smiling for an example. Do you have any suggestions?
