I need to make a website that is a free video lending library. There will be no financial transactions necessary.

There will be a number of pages with images and titles of videos and preferably a checkbox so that visitors can check one or several titles they are interested in.

There needs to be a form where they enter their name, address and phone number. When this is done a message needs to be sent to the webmaster with the personal details and the selections they have made so that he can arrange delivery.

I would like to get some suggestions on how to do this please. I don't know whether a database is necessary or whether html coding will be the best way, or if there is some other method. It needs to be simple to construct and logical for visitors to use.

You need some kind of serverside code to handle the form processing and having a database to manage the video titles etc would be pretty much essential.
