Hi,

What should I add to my .htaccess to tell my server to serv all .php and .html documents as:

application/xhtml+xml in UTF-8?

find this line in your htaccess
Code:
AddType application/x-httpd-php .php
and append .htm .html to it

sorry I mis-read your question

why are you asking the question?

xhtml/xml is not a server side process it's a client side, the doctype (DTD) and meta charset are there to tell the client how to render the HTML code.
