What is the platform of your server? you need an action to the form, IM asking about the platform cause you need to decide if you are doing a PHP file or an ASP perhaps, there are many other languages but you can find a working script for what you want to do.

Wufoo is a nice option to begin with. KIDJOE is right about the platform and technology selection. You hosted your website @ freewebs.com kindly check with them which scripting language they support i.e. asp, php etc
