I want to add a link on my website that will bring up a simple form that people can fill out to request services. Can someone give me the code for that?

seriously though we need a bit more information to help you with that you probably won't find anyone to write the form for you but you can do it in dreamweaver, frontpage, or whatever editor you're using if you're using a text editor check out http://www.htmlgoodies.com/tutorials/forms/ for information on making your form
