Hello all, I am trying to put a form script into my current webpage which is http://gcloan.com/default.htm but the problem that I am running into is that every time I put the form script onto it the content of the current website (as seen with my provided URL) moves all the way down and only part of the form script shows on the top left... the form script I'm trying to input is the following:

This same form can be seen on http://alliancedirect.net on the top left. In addition to this script, you see on the alliance website how an image is below the script as I included hotspots into it with fireworks but they look as one.

Also, on the gcloan.com, on the bottom left it continuously says that there is an error on the page which is shown on the bottom left of the page with the little error triangle ! and gives the following errors:

If anyone can help me, particularly with the scripting problems it would be highly appreciated. Thank you so much for your time and I'm glad to be a part of this excellent forum.
BTW, I am using Dreamweaver CS4 if that information makes any difference when someone is hopefully trying to help me. Thanks again!

Install the developer toolbar for Firefox and use the error console to see where you have gone astray.
