Hello, I have a client who wants a request/response form on their site like this: http://www.culligan.com/wateranalysis.cfm

They would also like to be able to save the collected data in some sort of database [are there different kinds?]

I'm a print/web designer/illustrator with medium HTML/CSS/Flash/JavaScript skills. I thought forms were built in JS, that's why I'm posting in this forum.

Can someone please just outline, in point form, the steps I need to take to deliver a form like this to them?

With no programming knowledge if i were you i would go this route:
http://phpformgen.sourceforge.net/

Its basicly a form generator and you can choose to have the form send via email and submit into a mysql database. Check it out.

This most likely belongs in the php forum.

Thank you capetek. I checked the link and I don't have a linux or apache server, but I appreciate you pointing me in the right direction. The client has his site hosted on Yahoo! and they do offer PHP, Perl and MySQL services and programming 'aid'.

I'll try that route and if I'm unsuccessful, maybe I'll hire you to create that part of the site. I went to your site... very clean, attractive and informative.
