I wasn't going to reply to this post because I fear my advice might not be very helpful to you.
I used to use phpBB for a fairly popular community that I ran. After a couple of years, however, I had to move to another platform ( Simple Machines) due to the sheer volume of spam I was receiving.
Without blowing my own horn, I'd say I'm a pretty competent php developer - I even wrote a few mods for phpBB back in the day, but I couldn't find any good way of combating the spam short of requiring my approval for all new members.
I guess what I'm saying is that phpBB is an old project, and has some pretty fundamental shortcomings when it comes to spam.
