Can't find the error in Guestbook script

I downloaded this beautiful guestbook script, but I just can't make it work. I did all the necessary configrations about the database and tables. But the comment box doesn't show up for some reason. I'm gonna blame it on the codes, but I don't know, they look so right. I must have missed one tiny point, just can't find it. Can you please help me and show me what I'm doing wrong. Well I have installed Apache, PHP and MySQL on my computer and I always test PHP scripts on the local server. I don't think the local server is any different than the other ones. But I tried to run the script on the internet anyway. Found a free hosting with PHP and MySQL services and tried it there too. Same result though. No comment box. Maybe check the script configuration?
