I have this poll on my website now:

And I followed these instructions carefully from:

But I don't know were I have gone wrong, because something isn't working right

Oh, you need to make your file .php (or equalivant via htaccess).

Check you have PHP installed on your server and access to a MySQL database also ^^

And yes PHP is installed on my server and I have access to a MySQL database

If you do have PHP installed then changing extension is all you'll need . It looks like you've got it figured out now.

But does anyone know how to actually add options for a user to vote

I can't see were to add them at all
