The only thing is I want to have the following information on there.
Name*, Email Add*, Business Name, Contact Number*

Have a Message box for messages same as current box on page.

Title for the following:
Im Interested in.
I want to have the following in a check box option.
Web design, Graphic design, print work, web updates, general enquiry, SEO work.
The ones with * are compulsary.
Please I need the form asap and must fit in the sites size which can be seen here http://www.swiftproductions.com.au/contact_us.html -
