Well, the first question you want to ask yourself is: why use PayPal? What I would do is try to convince my client(s) to use Authorize.net (an online payment gateway), since PayPal takes such huge cuts out of transactions. You probably want to do some research on gateways, and how they work.
Once you've got that, I would recommend that you use a pre-made shopping cart like ZenCart, or Drupal's add-on: Ubercart. Not so much ZenCart actually, seeing that it's mainly made for big businesses and it's overkill for anything that's smaller. And then you could always do a quick google search, and find something that fits your needs. There's a list of certified POS systems on Authorize.Net.
