Hey guys, I have a .pdf and a .doc version of a form with fields (simple radio, check/text boxes) that match those on an HTML form page.

I was wondering if anyone could point me in the right direction with populating either the PDF or the Word document using a php script? I've attempted to take a route through FDF files but unfortunately that plan did not come to fruition.
