Oh i see.You need to follow this steps:

1) Buy an expensive SSL certificate and install it.
2) Build your form and make it to insert the data into a mysql data base, do not send the CC data via email it is very unsecure.
3) Before you can start using your new built form you must make sure your server doesn't have any security holes. Installing newest software might fix this.

ok thanks. I have looked around for SSL cirtificates. They are indeed very expensive and i would like to ask a few questions.

1) if i bought one can it only be used on one website or anysite on the server?
2) Where is the best place to buy them? (For security and cheapness)

Thanks

You can get an SSL cert from RapidSSL.com for $49 which isn't too bad. You also need a dedicated IP address in order to use one. Make sure you can get one from your host (usually costs extra)

For a cheap SSL Cert, you can only use it on one domain. You'd have to by a wildcard certificate if you wanted to use it on multiple domain on the same server.

Some web hosts offer you a free shared ssl certificate. (we do for our customers), you can use that temporarily if possible. Ask your host.

SSL certificates are issued for only one host. For example "www.site.com" or "secure.site.com". A SSL certificated issued for "site.com" will note ensure the identity for "www.site.com". Or you can buy a wildcard SSL wich is almost 500$ / year and will ensure the identify of any subdomain like "*.site.com".

Good luck!
