I got to make a form of registering new visitors in my site with the following fields.

• Company Name
• Company Address with country and pin / zip
• Telephone number with country code and area code
• Fax number with country code and area code
• Email address
• Contact Person
• Designation
• Area of interest (options – Export / Import / wholesale / retail / buying house / joint venture / agency)
• Product of Interest (options – Small leather goods / Bags / Fashion / Utility / Business accessories / travel / leather / customized selection)

I'll be using MySQL & PHP....
