You can try, but make sure you change the website you apply with.

You probably won't be able to get back into AdSense though.

Your best bet is to contact Adsense and let them know what's up. Be polite, yet persistent.

You dont need any details other than anme and postal address if you are outside of the US as we don't need to pay tax etc.

My suggest is try with different Name and with different domain.

That's certainly not true. I'm able to get responses from them on anything and specifically know people who have had their account re-activated after contacting Google and letting them know what the problem might have been.
