I think the only thing that you are required to have is a way for the reader to opt-out of receiving it. If you have that, I think you'll be fine legally.

You also should include a physical postal address in the body of the email as well. Currently, I'm using an autoresponder which automatically puts that in at the bottom of every email. I've also seen others who put in a message like "You are receiving this email because you signed up for my list on [Date] from IP Address [IP Address]...yada yada yada" but this isn't required by law and might actually be overkill.

Basically say this is not unsoliciated and give an opt out option. I'd also include a contact option.

Yes, I believe it is now federal law that you must provide a way to opt-out.
