I sometimes get complaints about specific sites or people saying the size of the ad is too big. Do you ever have a problem with this? And how do you handle it?

Some ads are large ,

and we choose only the 468 x 60 pixels type banner.

Yeah. I get that from members on some of my sites. They complain that there are too much ads. And I usually reply that I have to pay the bills.

Agreed.
You need to explain to them as most people don't like the ads,especially after 1 or 2 times.Maybe using some smaller one except the top banner.
Finally, each user should support the site , for example of small donation.hone:

To expand on that, if users don't want to see ads, you should offer some kind of premium membership where they pay a certain amount and get to see no ads, in addition to other features.

The other option is to offer ads that reside inside the content of your site; like a contextual pop-over inline ad. That way, there is no site clutter and the ads only appear when one of your site visitors shows an interest in the ad.

I thought you decide what size you want?

All ads will expand over content if not placed properly...

I've stopped using Adsense much now anyways; the service is really not getting better than before.
