the best url structure is without urls... It is also called "url rewrite". Just search for that phrase on google. You'll get tons of answers.

Generally, you can theme your categories.

It doesn't matter if your last one is a folder or a file.

What matter is, though, keywords in the URL, no parameters and the least folders (/) and the shorter the URL is, the better.

I agree with you It doesn't matter that the last one is folder or the first one and the best url structure to write is always in the form of dashes for ex. i-am-a-good-boy.html

Generally, it doesn't matter if the URL ends with an extension or not.

Most often, you will have multiple files in a folder, so there will be an extension, anyway.

However, you can have the main page of the product be called index.php/html/asp, and you can then direct folks at site.com/category/product easily.

Mostly, it doesn't make much sense, it is just that the less folders there are, and the shorter the URL is, the better. Most probably, only the file depth matters.

Each site is differently, there is no absolute answer. However, the directory only structure looks much cleaner.
