I don't think your keywords have really been researched properly, you appear to have guessed rather than tried to choose keywords with high search volume and low competition - I would reccomend you start by doing a lot more research on your keywords. The Google keywords tool is quite useful, also wordtracker, maybe have a look at them.
Also, I noticed that you have tried to optimize your title and meta description - all well and good, but you have just stuffed them with keywords rather than thinking about the user. At the end of the day, they are (probably) going to be the things enticing users to click on your link from Googles SERP's. There are better ways to do this, ways which you can still enjoy keyword density and proximity in your title and meta description, but appeal to the user rather than just the robots - which should always be your priority really.
