In my opinion, join more and more forums, and post about your expertise and your requirements for the link exchange. So that people will contact you. Don't do his in the blog commenting area, as people do not bother to read the comments.
Do a search for relevant sites in your industry/themer/whatever. Then you should actually check out the sites, some of them will have set link-exchange policies and will tell you among their pages how their system works. If not, get an email address from the site and simply ask the webmaster for an exchange. It is always good to add the URL where you can put their link on. You can even find software out there that will help you find relevant pages and get the email addresses.
What I did in my LE format is, I shortly introduce myself an what site I am promoting, then proceed to the main reason which is to offer link exchange. Or if possible, offer your partner that you add their site first before yours.
You can also add resource page on your site's home page where you can list other sites from whom you have exchanged links and if someone sees it then they would contact you as well.

some seo tools offer automatic submission for inviting link exchange.
however you can make your own database of sites you want and make an email that will also be your template then start sending your invitation.

Whatever you've been reading mate, these are not 100% true. Link exchange is good if you have a valid reason to have that much links on your site. Otherwise, you'll stand out like a 'link Christmas tree' with no or little effect on your site on SERPs.
Google specifically, don't like you to exchange links unless you have a valid reason. This includes three way or four way link exchanges as well. So if you really want to exchange links, do it. But do it with caution.

1) Search on Google and send them link exchange request
2) Join link builder forum and request them
3) Be online on chat and frequently add some link builder so that you can offer you link
