I've been dealing with increasing spam recently (big surprise, I know), and I was thinking of adding a confirmation email feature in order to register. I already have a captcha, but I'm sure its not all that tough for the real spammers to get through.
So basically, what I'm asking is this: Have any of you noticed a great decrease in spam when adding a required confirmation by email feature to your site?

Every little bit helps. I wouldn't operate a forum without it. Keep in mind even after you do implement email confirmation, the bots are sophisticated enough to also set up a bunch of valid email accounts and can confirm emails easily. If you are using vbulletin, there are several anti-spam mods you can get at vbulletin.org. Two I recommend are Check4Spam and Stop the Bots. Together, they have eliminated 99% of spam on my forum.

You should defo confirm the email addresses, it does help with a lot of spammers, as most spammers are lazy and cant be bothered to do this !

I use email confirmation and the only thing that really works is to keep an eye out for the spammers and keep deleting their accounts every time they show up. I have one idiot in particular who sends spam to the admin. I delete it and a week later he is back. You would think he would be smart not to send the admin his spam, but every week he does. Once you let a few spammers take hold, the rest move in and destroy the place. They must be wiped out the minute they show up.
