Very simple:
Pre-installation:
Make sure *BEFORE* you run the install you have the following information already set, and if not I could assist you, but it's pretty simple to do.
Database server name: You can find this in the control panel of your hosting account, and if you are testing this on your own test server, then try localhost
Port: Leave blank, it will default to the correct port as it states so
Database name: Create a database for this script in MySQL...Located in your webhosting companies Control Panel. Once you have created the MySQL database, note you need to also create a username and password for the database as well, it should be easy for you to figure that out, then take note of the database you created, and the username & password you created for it, then enter in the setting box you posted above.
Whhhhhhew! That should finally do it.
