They seem to only have shared hosting and reseller hosting. Proxy site will be banned in no time if hosted in shared hosting.

Not sure about that :s I believe that CGI proxy is CPU intensive while PHProxy is bandwidth consuming. I have 2 sites running at VPS Hosting by Hostaga.com. If you are worrying about speed, you might want to try dedicated server. Hmm, I don't know why would a dedicated server not allowing you to host proxy site as only your sites will be hosted (so there is no affecting other people). I know that the hosting providers that I have asked, most of them only allow proxy sites on dedicated server.
You can check out Hostaga.com if you want. The support is great. I have 2 small proxy site hosted by them right now. They also have a lot of coupons going on right now (Free bandwidth increment, 50% of cpanel or management, $40 credit etc.)
You will need a minimum RAM of 256MB. Though I believe a lot of people going for 2 GHz (But those are expensive  )
You can check out http://www.managemybox.com/ I have seem people hosting their proxy sites there. Though I have no experience with them. But their dedicated plan seems to be very nice. Since people hosting their proxy sites there, so I assume they allow proxy sites.
