Photoshop for images
HTML Kit for editing and light FTP
Notepad 2 for editing simple files
Filezilla for FTP
I have plenty of other programs like Flash and Illustrator and Freehand and a ton of editors, but the programs above are the ones I use most frequently.
Added: Everyone below is reminding me I should have mentioned Firefox. How could I forget given it's the program I use more than any other. There are so many extensions available to help development I don't think I could live without it anymore.

Photoshop and Illustrator for graphics and photos
TopStyle Pro for coding HTML and CSS
Dreamweaver - once in a while, good for site management
FTP Voyager for well.. FTP 
Flash for animations
I also use several Firefox extensions for debugging and validation.

Graphics: Photoshop and sometimes Freehand if I need to create a shape from scratch. Flash for interative animation.
HTML & PHP: PSPad (use for ftp editing too) and rarely I use Topstyle Lite for CSS
FTP: SmartFTP Client (or use Windows Explorer for simple FTP)

Photoshop and or ImageReady for images
Notepad 2 for editing most html or php files
The 1995 version of ws_ftpLE for uploading (old friends are best sometimes)
And every now and then (sigh) I resort to FrontPage.

Apache/Php/Mysql (Dev/CMS Coding & Integrations Mainly)

FireFTP/CuteFTP (FTP's)

I am aware that PS/DW are considered industry standards, its my personal choice as to not support Monopolizing Corporate Mongers (When it can be avoided) & aid the smaller Software companies to compete (Thus aiding in new employment opportunities & market flux through expansionism)
