I would like to put a basic mail form with a few required fields on my subdomain.

Basically I am looking to collect the feilds on the form as a possible lead that will be mailed to me.

Any suggestions, like where to go to find a form that i can customize or make on my self.

If you have fantastico there is a script called phpforms that will allow you to make a custom form very easily.
