PDF form site integration

I have a requirement to be able to place an online pdf form, not just a document, but a form where the user can insert their information directly into the form.

Then have that form submitted to the server. I need to be able to extrapolate all of the data inserted into that form into a mysql db.

I know this can be done, just cant find the right location of the documents on the web.

Any information on this would be greatly appreciated.

The form will have to be in HTML, submitted by the user, stored in the database using server side code.

i could do it in php/html/mysql etc but I could have sworn I had seen somewhere where the pdfs could be exported into the mysql db's etc.

As a BLOB they could be stored in the database. But you would need the server to create the pdf file from user supplied data POSTed via HTTP

so I could take the php/html form and create the pdf file and store it as a blob. ok that would help a couple of things, but not the big enchilada. I am heading towards an administration interface where they can dynamically build the "forms" I was hoping to just use adobe to build them.... sounds like just html/php etc.
