Hi:

I'm creating a simple HTML page with Dreamweaver.
The page has some Radio buttons and some textboxes,
I already have the document saved on my PC as a HTML document.
When I open the document and select the radio button options for the questions that the document has and fill out the text boxes with the information and I go to FIle and save as ..
Once I open the document its comes up like if it was the first time I opened it without no editing...

Do I need like a save button or something so it saves the changes I have made ????

Nope you need a web server, some server side code and probably a database to collect the information.

how do you make the database collect the information a user sends? Thats my only confusing!!

You code the data collection routines and database insert in whatever server side code your host supports.
