Converting PDF form to HTML form

I have a client that currently has an editable PDF that allows the user to fill out the info and email an XML data file with the form contents. She then simply has to open the original PDF form in Acrobat and import the XML file into the form.

It's not to difficult a process but she has a hard time doing it. I need to simplify the process for her. I figure just using a regular web form that sends the contents in an email should suffice. Only thing is, there's a lot of stuff to put on the form.

You can see the form by going to: http://gsmusic.com/vsm/pdf/Jan%2006%20order%20form.pdf

She wants to do away with the auto calculating fields, so that's not a problem.

Any recommendations on easily converting from the PDF to an HTML form? Or am I going to have to sit here and remake the entire form from scratch?

Thanks in advance!

I was going to tell you that I thought you needed to just code the form on your own then I looked at the pdf and can see why you wouldn't want to.

I did a search on Google for 'converting pdf to html' and it came back with a lot of results the first being this page from the Adobe site for PDF conversion which looked pretty good. I hope that helps. if not there were a lot of results so maybe one of the other sites will have what you need.

Yeah, it's a doozy. There's a lot in there.

I tried the Adboe conversion tool and it chokes on the conversion. Would have been nice if it would at least convert to plain HTML then I can add form functionality to it.

I looked a little deeper in Google and found 2 software based tools that claim to convert PDF to HTML. We'll see.

The current PDF was created in Adobe LiveCycle Designer, so I'm not sure if that's going to cause problems. The form isn't just editable, it also has formcalc functions. That might be the issue.

Thanks for the tip though, I'll update if I have any success with the stuff I downloaded.

No luck. No conversion tool would complete a PDF to HTML conversion with the form. I think it has to do with the fact there's an XML file attached because of the FormCalc functions. I don't know LiveCycle Designer well enough to remove all the functionality.

I broke down and just remade the form from scratch.

you know what i found this post and i am having a problum, i dont know if it is the same thing or not... i put a post in the HTML area... here is my message...

Quote:
I scanned in a Permission slip and edited it in Acrobat. i made the image into a form and i want the form to be emailed with the information the user inputs...

My instructer was spoced to help me but never did lol

here is the thing.. i can't find where to put the SUBMIT button since it is not in a .HTM file.. it is a direct link to see the PDF File...

my question's are

1. Where can i add the Submit button

2. and when the button is added is there anyway it can be emailed with out any sort of external app. or making the people save it and open their Web browser email and add the attachement.

I understand you can add a button to the image its self, but when the e-mail is submitted i dont want the button when u print it out
