Use this as your <select> section of code:
Code:
            <select name="select">
              <option value="Internet">Internet</option>
              <option value="Print Ad">Print Ad</option>
              <option value="Referred">Referred</option>
              <option value="Other">Other</option>
            </select>

Thanks a bunch. I tried what you said and I am still getting no results. I wasn't the original designer of this site. There is a feedback. html file in the root folder as well. What is this? When I click submit a page shows up describing what you sent in your feedback. The info from the feedback.html file shows up instead?????? Does this make any sense? Is the data going through another .html file before it is sent to an email? Thanks for any help. I'm about to pull all my hair out!
