Need someone to create a Mini Site with a form. Need something close to this:

This is for mortgage. Need something simple, clean, professional, sleak...better design than this site but general overall idea the same.

It's going to be hard finding responses for less than $100 in a short amount of time, especially as you're wanting PHP/MySQL form creation as well as the design and implementation.
